Key Responsibilities
- Work respectfully with clients' families, support workers and other providers to build strong relationships
- Arrange individual services shaped by each client's goals and health conditions
- Assess each client, write their care plan and keep it under review
- Decide with client safety, wellbeing and quality of life in mind
- Oversee the Support Workers
- Guide the team towards consistent, continuous service
- Keep client budgets under watch and manage them
- Comply with legislation, funding guidelines and internal policy
- Write progress notes accurately and on time
- Go through notes from support workers, suppliers and allied health professionals
- Admit new clients through intake assessments and meet and greets
Requirements & Eligibility
- Aged care, home care, community services or health experience
- Assessment, care planning and case management ability
- Independent working and the confidence to manage competing priorities
- Excellent at communicating, building relationships and leading people
- Working knowledge of My Aged Care and the Aged Care Quality Standards
- A driver's licence and your own vehicle
- Willingness to travel across Sydney
- Bilingual or multilingual ability is a strong advantage
- A background in nursing or allied health is beneficial but not required
